The purpose
of the Jefferson County Health Department Home Health Division is to provide
and coordinate physician-directed, individualized, comprehensive, and safe
skilled nursing and home health aide services to residents of Jefferson
County.
The Home
Health Division has been established to meet conditions of participation in the
Federal Health Insurance Program of Medicare.
It also meets conditions to provide care, which is reimbursable by funds
from Medicare,Medicaid, Veterans
Administrations,Lifetime Resources, and private pay patients.
1. The
primary functions of the Home Health is to provide skilled nursing services and/or home health aide services on an intermittent visiting basis in the individual's home.
2. Staff
is directed by the Director of Nursing and is under the supervision of the
Supervising nurse.

Nursing services
a. The
R.N. or L.P.N will
perform duties as ordered by your physician.
II.
Non-skilled nursing services
a. Home
Health Aides are to be assigned when the patient needs personal care.
b. Homemakers
are assigned when the client needs help with light housekeeping, meal
preparation and errands.
